How to order HERmark breast cancer assay
1) If you need an account set up call Monogram Biosciences at 1-800-777-0177 or click here to open an account.
2) Monogram will send you shipping kits and Test Request Forms.
3) A TRF must accompany every patient specimen.
- A Medical Oncologist/Licensed Health Care Professional may complete the Pathology Preorder Form (download here) and send the form to Pathology.
- Pathology can complete the Test Request Form and follow the instructions in the shipping kit.
4) Results are typically reported in seven days from receipt of the specimen.
